Abstract: There is ample evidence available to indicate that people with enduring, serious mental illness experience poor physical health outcomes, however there is a deficiency of programmes or research that demonstrates possible solutions. This paper describes a model utilised in the ACT within an adult mental health team that has shown encouraging outcomes.
