Abstract: The Warrnambool and District Base Hospital Adult Community Psychiatry Teams provide a comprehensive, specialist psychiatric service in south western Victoria. The services have moved from an isolated model of psychiatric care to a community based model of integrated mental health care which is delivered as close as possible to the client's environment. Comprehensive assessment and treatment is provided with an emphasis on home based care, case management and an individual service plan for every client. Both clients and carers are encouraged in all areas of management and the teams liaise closely with all general health care providers and the community served. Description of Facility/Organisation: The Warrnambool and District Base Hospital Psychiatric Services Division Community Adult Psychiatry Teams are located in south western Victoria and service an area of 25,000 square kilometres with a target population of 62,000. The teams are located in the city f Warrnambool and towns of Camperdown, Hamilton and Portland.
