Abstract: Nelson's Mobile Community Team is a component of Con-Serve (Comprehensive Consumer-based Service Plan) developed to meet the worldwide challenges for mental health services: effective community care. The team is dedicated to community based mental health, providing 24 hour crisis intervention and an assertive outreach Service for long-term clients. Their primary focus is prevention - being proactive, optimising accessibility, reducing hospitalisation needs, increasing rehabilitation choices and improving family and community knowledge. reviews two years on show it is achieving its objectives - offering high quality community care, ensuring effective service delivery, efficient resource utilisation and customer satisfaction. The biggest consumer complaint: not enough of them! Description of Facility/Organisation: The Team comprises 10 multiskilled staff providing crisis intervention and assertive continuing care for long term clients in the catchment area of 70,000. It has 500-530 referrals annually, approximately 250 of which are continuing care clients. Each member carries a caseload of 25-30. Referrals can be from GPs, Community Agencies or from clients/families/whanua.
