Abstract: The North West community Mental Health Service is a state run area based mental health service providing clinical, case management and rehabilitation services to the cities of Moreland and Hume. The North West Area services have substantially refocused the service system into collaborative structures with the general health services and the wider community. Key initiatives have been the development and implementation of needs assessment tools and a sophisticated case management system, Community Liaison in Primary Care Psychiatry, Religious Groups Community Development Project, increased supported accommodation and consumer initiatives. Description of Facility/Organisation: The North West Community Mental Health Service is a State run are based mental health service providing clinical, case management and rehabilitation services to residents of the cities of Moreland and Hume. Services are provided by a range of multi-disciplinary teams. The catchment area has a population of approximately 200,000 and comprises inner suburban, suburban and semi rural areas.
