Abstract: This paper presents the implementation of a service initiative developed within Counties Manukau (South Auckland). The new service, titled Personalised Solutions: Community Living Service (CLS), was established with the intent of supporting people to live within their own homes and contribute to communities of their choice. The CLS was set up to respond to gaps in mental health service provision and to enable people to pursue their ambitions in a more meaningful and self-determined way. This paper will introduce the concept and design of the new service and will explore the collaborative and evolutionary approach taken to service implementation. It will lastly discuss the hoped for outcomes and the intended approach to evaluation.
