Abstract: A lack of effective discharge planning may result in blockages to a consumer’s transition from hospital back to their community; and reinforce the ‘revolving door’ syndrome. This presentation reported on a project undertaken by the Consumer Consultants at Southern Health Mental Health Service in Victoria. Initially aimed at mapping current discharge processes and identifying areas for improvement, the project was later modified to provide data to allow Southern Health to develop and put in place a discharge strategy that ‘works’ across all their inpatient facilities.
