Abstract: The Logan Mental Health Service is an ACHS accredited integrated mental health service serving a catchment population of 150,000 people based in Logan City, Queensland Hospital Psychiatric Unit and the Woodridge Adult Mental Health Clinic. Since that time, a new service delivery model (matrix) has been introduced which ensures continuity of care. Also since 1994, a range of new services have been introduced, including an Extended Hours Service, a Mobile Intensive Treatment Team, an Emergency Phone Advisory Service, a Rehabilitation Service and a Hospital Consultation Liaison Team. The Service has also facilitated a Consumer and Carer Group and have begun developing a range of linkages with other local service providers and non-government agencies. Description of Facility/Organisation: The Logan Hospital Mental Health Service consists of a community based service at Woodridge and a 23 bed inpatient Unit at Logan Hospital. The service model provides for continuity of care with the same clinical team providing treatment in either the community or the inpatient unit. Additional services provided include an Extended Hours Service, a Mobile Intensive Treatment Team, a Rehabilitation Service, and Emergency Phone Advisory Service and a Hospital based Consultation Liaison Service. the catchment population is approximately 150,000 people with an annual budget of $3.8m.
